Advance Care Planning: Removing the Barrier!

Course Description: Advance Directives are encouraged for all however, few people actually talk about them, let alone complete legal documentation regarding their wishes. Research indicates this absence of advance care planning has led to numerous unintended consequences including things like: treatment that was not desired by the patient, delays in care, legal and financial complications, increase in spending and hence decrease in satisfaction with healthcare in general by patients, family members and other stakeholders.
Learning Outcomes: The learner will be able to identify the difference between Durable Power of Attorney, Guardianship, and Conservatorship and be able to name one way to implement what they have learned in this course.
